Brazil Application Form
The Consulate of Brazil requires a visa application completed
online, found at: https://formulario-mre.serpro.gov.br/sci/pages/web/pacomPasesWebInicial.jsf

Copy of Driver’s License
You must provide proof of your
current address of residency. The document must be either a notarized copy
of your driver’s license or a notarized utility bill
Your notarized copy must display your home address as it appears
on your visa application form
The driver’s license must be notarized in the state it was
issued and display the notary’s commission number and expiration date
The driver’s license cannot be expired
If you must submit a utility bill the
consulate requires electric, gas, or a water bill; phone and cable bills are
not accepted
Proof of Travel Arrangements
You must provide proof of travel
arrangements for the duration of your trip. Details of your intended
arrangements must:
Include a copy of an itinerary or e-ticket that displays your round
trip travel and entry and exit dates for your destination
Show the traveler’s full name (as printed in his or her passport)
Curriculum Vitae
You must provide a notarized
copy of your most recent Curriculum Vitae
Additional Documents
You must provide notarized
copies of the following documents:
A two signed copies of a “Statement of Commitment”
that the applicant will not engage in any activity in Indian areas without the
proper authorization
by FUNAI – National Indian Foundation
A copy of applicant’s ordination certificate or diploma and
school transcripts for theological studies
A copy of the Brazilian organization’s registration in the
National Registry of Legal Entities (“Cadastro Nacional de Pessoas
Jurídicas”)
An empowerment certificate issued to the Brazilian religious or
missionary organization’s head officer
A copy of the charter and/or bylaws of the Brazilian religious
or missionary organization
Invitation Letter
You must obtain an original letter of invitation from your
host. The person you will be visiting is responsible for obtaining the
invitation. The letter of invitation must:
Be signed by organization’s head officer or by an individual
qualified to legally represent the organization
Must invite the applicant to go serve in Brazil and list the
applicant’s name as it appears in their passport
Declare who will take financial responsibility for the expenses
that will be incurred while in Brazil
List the specific travel dates
Guarantee that the applicant will not engage in any activity in
Indian areas without the proper authorization by FUNAI – National Indian
Foundation
List the local Brazilian address and name of the Brazilian
contact person
FBI Clearance
All visa applicants who have lived in the
United States within the last 12 months must provide an FBI Clearance. The
clearance must:
Be issued within the last three months
List the applicant’s full name, address, and the last four
digits of their social security number
Certify absence of criminal record
Be legalized by the Consulate of Brazil
Be issued by FBI headquarters in West Virginia or be obtained
via FBI-Approved Channels
Birth Certificate
(You must provide your birth
certificate showing both of your parents’ names. The certificate must:
Be original or a notarized photocopy
Be translated unless the original document
is in English, French, Portuguese or Spanish)
