Passport
You must provide your actual signed passport, including one copy
of the personal information page of your passport. Your passport must:
Be valid for the next six months
Have at least one blank visa page (amendments and endorsement
pages cannot be used to fulfill this requirement)
Not be frayed, torn, separating, or altered in any other way
Photographs
You must provide two colors, passport-type
photographs, be taken within the last three months against an all-white
background and printed on high-quality photo paper; be identical; be two inches
by two inches; provide the full frontal view of your head with your
Face centered in the middle and displaying a neutral
(non-smiling) expression. You may not wear eyeglasses or headwear except for
religious purposes.
Visa Application Form
You must provide one completed copy of the visa application form found in this kit. The application form must:
Include answers for all fields
Display your full name as it appears in your passport
Be signed
VISA QUESTIONNAIRE
The government of Bolivia has
instituted an online visa application form. This electronic process is
extremely complicated and has portions that are only
Available in Spanish. Orbit visas will complete the entire
process on your behalf.
Simply fill out the Bolivian Visa Questionnaire included in this
kit and enclose it with all additional documents listed on the requirement
checklist – we will take care of the rest.
Once one of our visa experts completes the online visa process,
you will be contacted via email and receive a copy of the online visa
application that
you must sign and return to us.
As soon as we receive the signed copy of your visa application
form, we will submit all of your documents to the Bolivian Consulate and your
visa will be processed.
HOTEL CONFIRMATION
You must provide a confirmation of your hotel accommodations.
Your confirmation must:
Include your intended dates of stay
Display your full name
PROOF OF SUFFICIENT FUNDS
You must provide proof that you
possess sufficient funds. Your financial details must:
Include consecutive bank statements for last 6 months
Display your full name
Proof of Travel Arrangements
You must provide proof of
travel arrangements for the duration of your trip. Details of your intended
arrangements must:
Include a copy of your most recent bank statement
Display your full name (as printed in his or her passport)
INVITATION LETTER FROM FAMILY OR FRIENDS
If you are visiting family or friends in Bolivia, then you must
provide an invitation letter. The letter must:
Include the full address and contact information of your host
Include the dates of your stay
Include your full name as it appears in your passport
VACCINATIONS MAY BE REQUIRED
Some countries may require vaccinations if your immediately
prior point of departure is from countries deemed a health risk. Please go to
http://wwwn.cdc.gov/travel/contentVaccinations.aspx. for the
most up to date health requirements for each segment of your trip. You must
provide a yellow fever vaccination certificate if you are traveling to or from
a country deemed a health risk for Yellow Fever.
